Are you a Professional Performance Artist and want to create art live at the Art, Dine and Design? We are seeking Artist that will create art live at the event and mingle with event guest.

Please fill in the following questions for The Northwood Village Art, Dine and Design Performance Artist Entertainment Application / Information Request Form. This form is what we will use to advertise you creating live art and also use the information for your payment.

Your payment will be available the night of the event and be paid to you by from Hennevelt's Gallery and Gifts.

Please choose dates that you would be interested in creating live art and we will contact you on what is still available.

Event Dates run on a fiscal year of October - September.

The Northwood Village Art, Dine and Design is held every Last Friday of the month from 6 pm - 9 pm

Set up is from 5 pm - 6 pm please plan to arrive in enough time to start playing at 6 pm. Check in will be at the NW Corner of Northwood Road and Spruce Ave. Set up location will be determined at the time of your check in.

Note the Art, Dine and Design event is a rain or shine event. In the case of extreme weather, the event night could be canceled for the safety for all.

We suggest that you bring a 10' x 10' pop up tent. Your tent will be required to be weighted for safety.

You will also need to bring your own tables, chairs and supplies to create your art. All art created at the event will remain yours.

You may bring art to sell and collateral to had our to guest.

Your application will be reviewed and juried by the Art, Dine and Design Committee. and you will be notified via email. Please add HenEvent@gmail.com to help avoid our email from falling into your spam folder.

We will require you to email us an invoice for your services in the amount of $250.00 at least 3 days prior to your scheduled event date. Please email your invoice to HenEvent@gmail.com.

I grant permission to all the foregoing entities and persons to use any photography, motion picture, recording or any other records of me in this event for legitimate purposes.

I understand that all or part of the event can be canceled due to extreme weather.
