This enrollment form is required to establish a connection between the MyQ Community Mobile App and each individual user. The information obtained below will be used for the sole purpose of activating the Mobile App and issuing each user a link to download the MyQ Community Mobile App.

• Owners can register up to four (4) phones per household. (including adults and children 16 years of age or older)
• Each phone requires a separate registration from LiftMaster to obtain a download link
• Each user MUST have a unique email address to obtain a download link.
• Tenants can ONLY register phones for persons named on the lease. (verified by management).
