My dog is current on Rabies (if 6m or older) and DHPP vaccines or titers and free of all illness and parasites. I will supply a copy of my dog's current vaccines via email or at my first class. Initial: *

Please do not bring your dog to class if they are showing any signs of illness, including lethargy, coughing, sneezing, vomiting or diarrhea.

My dog has NOT/will NOT receive the kennel cough (bordatella) or canine influenza vaccine within the 7 days prior to first day of class. Initial: *

Kennel cough and influenza vaccine are NOT required for class with us. If your dog receives these vaccines, please ensure that they are given a MINIMUM of 7 days prior to class.

I understand that the class must be paid in full to reserve my seat and that all fees are non-refundable after my assigned class start date. (2-7 days before my class start date, I may receive refund minus a $55 cancellation fee. I may cancel or reschedule my class without fees if I give more than 7 days notice).

I understand the Real Life Training class is a manners program and therefore not appropriate for dogs with aggression issues. I certify that my dog does not have a bite history and is not known to show aggression toward people or dogs. Initial: *

I understand and accept that I am responsible for all actions of myself, my guests, and my dog/s while on Tails of Success property. Therefore, I release Tails of Success and their landlord, employees, instructors and assistants of any and all liability for injury to myself, my guests and my dog/s.
