2025 Giving Statement Request Form
Giving Statements are sent by email to every parishioner for which we have an email address. If you did not receive one, please check your spam folder first before requesting another letter. It was sent using the email address:

notification@ondemandmail.com.

For those contributions made through tax-exempt accounts, such as an IRA, they are NOT included in the Giving Statement. Individual acknowledgement letters are mailed via the USPS immediately after receipt.

Other contributions made by donating to school fundraisers, the giving tree, or poor boxes are NOT reflected in this letter. Cancelled checks are a sufficient verification for tax preparation purposes.
