Due to the outbreak of the novel Coronavirus (COVID-19), Sky Blue Aquatics is doing everything we can to protect you, our clients, our community and our staff. To this extent, Sky Blue Aquatics will be following the Cayman Islands Government Regulation guidance with regard to social distancing practices and sanitation. We ask that our clients disclose their health history and continue to implement these sanitation and disinfection procedures.

I, nor members of my household, have not experienced any of the symptoms listed above within the last 14 days.
I, nor members of my household, have not travelled internationally in the last 30 days.
I, nor members of my household, have not travelled to a highly impacted area within the United States of America is the last 30 days.
I, nor members of my household, do not believe that we have been exposed to someone with a suspected and/or confirmed case of the Coronavirus (COVID-19).
I, nor members of my household, have not been diagnosed with the Coronavirus (COVID-19) within the last 30 days.
Sky Blue Aquatics cannot be held liable from any exposure to the Coronavirus (COVID-19) caused by misinformation on this form or the health history provided by each client.
If I take any steps to make a claim for damages against Sky Blue Aquatics, its agents, employees or any other released parties, I shall be obligated to pay all attorney’s fees and costs incurred as a result of such claim
Sky Blue Aquatics is following these enhanced procedures to prevent the spread of the Coronavirus (COVID-19)

I agree to ensure that the below procedures will be adhered too at all times:

I will cancel my scheduled lesson if my child or family has any COVID symptoms and wait 14 days to rebook or as soon as they have a negative test we can resume
If I cancel a lesson due to COVID symptoms, I am aware I will be allowed a refund and will not be able to swim for 14 days or till I have a negative result. However, if I cancel within 24 hours due to alternative reasons I will still be charged
Instructors have permission to cancel any lessons if they feel like symptoms have not been reported
Instructors will inform you if they have to cancel any lessons due to their own health, or feel like they may have been exposed to someone with COVID symptoms.
Parents must watch lessons at a distance of 6 feet or more unless they are participating in the lesson.
No siblings or other family members who are not partaking in the swim lesson, may be near or in the pool during the lesson
Each instructor is required to wash their hands with hand sanitizer upon arrival and before departure. They will also wipe down any surfaces touched (gate latches etc)
Students will provide their own equipment if possible. Any equipment Sky Blue Aquatics provides during lessons will be cleaned, sterilized, and disinfected before and after use
Payments will be made online where possible. If you need to pay with cash or check, please put it in an ZipLoc with your child’s name on
Instructors will wear shields in lessons where it is necessary to have physical contact with the student.
By signing below, I hereby release and agree to hold Sky Blue Aquatics harmless from and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses, and compensation for damages or loss to myself and/or property that may be caused by any act, or failure to act by Sky Blue Aquatics, or that may otherwise arise in any way in connection with any services received from Sky Blue Aquatics. I agree to release Sky Blue Aquatics from any and all liability for the unintentional exposure or harm due to the Coronavirus (COVID-19). Sky Blue Aquatics agrees to abide by these standards and affirms the same
