This form is to be used by ONLY UOnline students to let the UOnline team know that you will be requesting an ODS accommodation upon accepting your offer of admission.

It is important that your UOnline Advising team is aware that you will be requesting an accommodation through the Office of Disability Services to ensure that you complete your required documentation on time and that you are aware of the timeline for review. This is only an acknowledgement form and not the official request for an accommodation.

You must complete this form completely within 7 days of accepting your offer of admission.

Our UOnline Advising team will reach out to discuss the process, deadlines, and required documents needed in order to submit your request through the ODS system correctly.
