LLPB Seller Payout Request
Thank You for Completing Your Payout Form!

A few quick reminders to keep everything smooth and on schedule:
• A W-9 form is required each year for payouts. No form = no payout.
• Items remain for three full months. After that, unsold items may be:
• Picked up (local sellers)
• Shipped back to out-of-town sellers (fees may apply)
• Donated



Payout Details
• Payouts are only issued in your 4th month of consignment.
• They are processed the week after the sale (not immediately).
• Payouts are not automatic — filling out this form is required.
• If you miss your 4th month request, your payout will be delayed until the next scheduled cycle after a sale.



Payment Options
• Choose direct deposit ($10 fee) or store credit. No checks are issued.
• Inventory must be picked up (or shipped/donated if applicable) before payout is released.
