My Ascension Registration and Permission Form
On Sunday March 26th, Parkview Baptist School and Church will partner together for the showing of "My Ascension." This documentary and panel discussion will cover the topics of anxiety, depression, and suicide.
My Ascension is a true story based on a suicide attempt that left 16-year-old varsity cheerleader Emma Benoit paralyzed but propelled her on a mission to use her painful experience to help others find hope and stay alive. We feel this will be an informative session, as well as, an opportunity to discuss these topics from a biblical and real life standpoint.
As a reminder, a parent must be in attendance if your student is in 7th or 8th grade. For 9th- 12th grade students, we request you complete the below permission slip.

The permission form below is required for all 9th, 10th, 11th, and 12th grade students to attend.
All 7th and 8th grade students MUST have a parent in attendance with them.
