DEADLINE for submission of this form is Friday, April 5, 2024. If you need additional time, please contact CME staff (see below for name, email).

SPEAKER FORM TIPS:
1) First step - please be sure to review the speaker/faculty policies. Complete policies and presentations information are available on the HAOPS Speaker Resource web page: www.bit.ly/HI-speaker24

2) Attachments up to 10 MB may be uploaded to this Speaker Form where you see an upload link. It will be simplest to upload from a device that has the correct document on it already. You may also send it via email directly to CME staff. If the document is more than 10MB or is too large to send as an email attachment, please contact CME staff to arrange alternate method.

3) A link will be sent in April for uploading your Presentation Slides and any Handouts. Please submit those by Friday, May 10, 2024.
