VIP TANNING OF EAST LANSING MEMBERSHIP FREEZE/CANCELLATION REQUEST AUTHORIZATION:
CANCEL TERMS:
Per my original VIP TANNING OF EAST LANSING EFT Membership agreement, I understand that I waive all previously earned and unused promotional tanning visits, and my tanning privileges with this membership cease effective the last day of my monthly cycle.
Per my original agreement I understand that all forms submitted after after 48 hours before my next scheduled billing date will be charged the regular monthly dues for one final month.
Cancellation form must be submitted 48 hours before the END OF BUSINESS DAY AND BEFORE YOUR NEXT SCHEDULED BILLING DATE.

FREEZE TERMS:
Memberships status changes must be made by the terms agreed to in your original credit / debit card authorization form. V.I.P. Tanning will not refund/credit any account that does not terminate by the agreed to terms.
As agreed, memberships that are already in a frozen status cannot be cancelled without going through 1 full billing cycle before termination.
Smart Pay Memberships can be frozen a max of 2 times per year (once every 6 months) for a max of 4 months per freeze. Certain Smart Pay Memberships must complete 3 drafts before freeze option is available. No exceptions. Please see store for details on your account type.
