I have read and agree to comply with the FA Respect Guide and Ware Lions Code of Conduct for season 2025/26. Ware Lions FC pay public liability insurance via the Mid Herts League/ Herts FA . However parents & guardians are encouraged to make arrangements for other insurance cover if they believe it is necessary.

In the event of an accident/ injury where the coach is unable to contact the named above, I give permission for the coach to sign the authorisation for any medical treatment/ procedure which may be required. I also consent to qualified first aiders offering treatment if required. Parents remain responsible for their children at all times. If you plan to leave the ground/ training facility then you must arrange for another adult to take responsibility for your child and you must inform the coach.
