-This voucher is for Ohio residents only. Each person may only receive ONE voucher code.

- The voucher is good for $80 off of the HiSET Exam. You will need to pay $13.75 for the computer based version of the exam, $35 for the paper & pencil version of the exam, and $61.25 for the at home version of the exam.

-In order to qualify for the $80 HiSET voucher, you need to complete all information on this form. Incomplete forms will not be processed.

- When you "SUBMIT" a complete form, you will receive an email within 2 WEEKS from tliska@polaris.edu with your voucher number and instructions for redemption.

- After you've received your voucher, schedule and pay for your test.

-Voucher codes expire on June 30, 2023, so plan accordingly. If you do not complete all 4 sections of the exam prior to June 30, 2023, you will need to pay full-price for future sections of the exam.
