To reserve your exclusive spot on this retreat a $1,500 per person non-refundable deposit paid by check is required along with the submission of this form.

Checks are to be made out to Sonia Lennartz and can be given directly to Sonia or mailed to 1967 Palmetto Isle Drive, Mount Pleasant, SC 29466.

No spots are held until payment has been received.

*If you prefer to pay with a credit card a 3% processing fee will be added to the total.

We want to make your trip hassle free and finalizing accurate paperwork is an essential step in an effort to ensure proper Transportation Safety Administration (TSA) documentation requirements are met and security issues are avoided.

It is the PASSENGER(S) responsibility to complete the following form fully and accurately. If you have questions about or issues with Passport or Visa requirements, visit www.Travel.State.Gov

All submitted information is secure, properly protected, and used exclusively for your upcoming reservation.

Please note:
Travel Insurance is required, and not included in the retreat rate. Flights are also not included in the retreat rate. Flight information and travel insurance confirmation will be gathered on a seperate follow-up form.

  • If not valid for at least six months beyond scheduled date of travel, you are required to renew prior to departure.
  • With better technology, foreign governments are checking passenger manifests with more scrutiny. To get a US Passport, you only need to prove you are a US Citizen with no *outstanding* warrants or certain types of convictions. Foreign governments may deny entry for past federal, state or local criminal history including DUIs. It is every traveling passenger’s full responsibility to check and verify any and all passport, visa, vaccination or other entry requirements for their intended destination.
