ACKNOWLEDGEMENT OF RECEIPT OF KEY/ FOB TO THE NISSAN FRONTIER FOR USE AT TULSA WELDING SCHOOL (TWS), 243 Greens Road, Houston, TX 77060
By signing this form, I agree to the following: I am receiving information on how to retrieve the Nissan Frontier Patrol Vehicle from the Lockbox, for use during my shift. When I am on my shift and using the vehicle, I am responsible for the key issued to me; I will open the lockbox at the beginning of my shift OR assume responsibility for the keys when relieving another officer. If no other officer is relieving me, I am responsible for placing the key back into the lockbox, after parking the ASI vehicle in the designated spot, behind the building, and also ensuring the gates are locked. While the key is in my possession, I will be responsible for up to $200.00 of the cost of replacing the Key FOB, if I lose or misplace this key.
