Klook Mobile Test Services - Appointment Booking

This form is ONLY for booking appointments exclusively for Klook Mobile Testing Services provided by TheTestingPro.

IMPORTANT NOTICES

1) Mobile Testing Services MUST be booked at least 24 hours in advance.

2) All bookings submitted will be processed by our MTS Concierge. Please note that our MTS Concierge Operating Hours are: Mon - Fri (9:00 AM - 7:00 PM; Sat/Sun (9:00 AM - 1:00 PM). Bookings submitted outside of the Concierge Operating Hours will be processed the following day.

3) Upon confirmation of booking, you will be sent instructions on how to submit individual personal particulars for a Notarised Digital Health Certificate (MTS HealthCert Request Form). You will be required to submit MTS HealthCert Request Form at least 12 hours before the time of your test.

4) Digital Health Certificate will also includes EU DCC compatible QR Code.

5) We strongly encourage you to check with us before booking if you have specific requirements to be indicated in the digital health certificate.

6) ADDITIONAL SURCHARGES

a. Long Distance Surcharge: $28
(For locations at Sentosa, Jurong Island, Changi Airport, Changi Village, Tuas Area)

b. Sunday & Public Holiday Surcharge: $28

c. Last Minute Booking Surcharge: $28
(For bookings made less than 24 hours in advance)

d. Off-Hour Surcharge (6:00 PM - 9:00 PM): $28

7) PAYMENT

All Klook Mobile Testing Services (MTS) surcharges must be pre-paid in advance. Upon submission of this booking form, you will be sent an invoice to make online payment. We only accept credit card payments (Visa, Master, Amex).

8) RE-SCHEDULING

Re-scheduling is allowed ONLY if requested more than 24 hours before scheduled booking time, and the re-scheduled booking cannot be more than 48 hours from original scheduled booking time. There is a $28.00 admin fee (per booking) for re-scheduling.

9) CANCELLATION POLICY

We highly discourage cancellation. A cancellation fee will be charged as follows:

• Cancelation more than 24 hours before scheduled booking: 20% of total booking fees paid
• Cancelation between 24 – 12 hours before scheduled booking: 50% of total booking fees paid
• Cancelation less than 12 hours before scheduled booking: 100% of total booking fees paid
• No Show / On Site cancellation : 100% of total booking fees paid

    Our Mobile Testing Services available daily from 9:30 AM - 9:00 PM.

    Bookings for all mobile testing services must be made at least 24 hours in advance.

    Bookings that are made less than 24 hours in advance are considered last minute bookings and are subject to a surcharge of $28.00 (per test). To make such last-minute bookings (subjected to approval), please contact our MTS team via email: mts@thetestingpro.com.

    There is a surcharge of $28 (per booking) for bookings on Sundays and Public Holidays

  • Test Location

    We are only able to conduct mobile testing services within the compounds of:

    1) Private Residences
    2) Offices with rooms or private areas
    3) Hotels Rooms or Function Rooms

    Please note that we do not conduct any mobile test services at public areas such as void decks, carparks, lobby, etc.

    There will be a LONG HAUL surcharge of $28 (per booking) for the following locations:

    a) Sentosa
    b) Jurong Island
    c) Changi Airport / Changi Village
    d) Tuas Area

    By submitting this form, you acknowledge and agree that:

    1. You have read and that you understand all of the above stated herein.

    2. You are aware that all bookings submitted will be processed by our MTS Concierge. Please note that our MTS Concierge Operating Hours are: Mon - Fri (9:00 AM - 7:00 PM; Sat/Sun (9:00 AM - 1:00 PM). Bookings submitted outside of the Concierge Operating Hours will be processed the following day.

    3. You acknowledge that your booking will only be confirmed upon receiving confirmation booking email from mts@thetestingpro.com.

    4. Where applicable, you have checked and ensured that the test which you selected is valid, accepted and within the testing window as a Pre-Departure Test (PDT) requirement by the country that you are traveling to.

    5. You agree to be contacted by TheTestingPro and its employees, through email or phone, for the purpose of this booking, if and when required.
