-This voucher is for Ohio residents only. Each person may only receive ONE voucher code.

- The voucher is good for $80 off of the TASC Exam. You will need to pay $14 for the exam.

-The FY19 TASC voucher is valid until June 30, 2019. This means that once you start the testing process, all five sections of the test must be taken prior to the June 30, 2019 expiration date. There will be no extensions or exceptions. The fee amount can be accessed all at once if you take all subject area tests at the same time. If you schedule the subject area tests separately, the reimbursement is $16 each.

- When you "SUBMIT" a complete form, you will receive an email within 2 WEEKS from agraham@polaris.edu with your voucher number and instructions for redemption.

- After you've received your voucher, schedule and pay for your test at https://ohio.tasctest.com/

-Voucher codes expire on June 30, 2019, so plan accordingly.
