  • Permission Agreement

I hereby give my consent for my child to be involved in all activities associated with Humane Society programming or field trips off the premises, as long as such activities are supervised by a Humane Society staff member.

I understand that photos or videos of my child may be taken during program time for program displays, staff training purposes, publicity brochures, newsletters, the Humane Society Annual Report, media coverage and any material and articles promoting the North Bay and District Humane Society. If I do not want my child’s photo or video to be used as in the above-description, I will inform a Humane Society staff at the time of registration to ensure my child is exempt from photos or video coverage.

  • Behaviour Management Agreement

The following behaviours are not acceptable and may result in the immediate removal of a participant for the remainder of the current day or the entire program.

Unacceptable Behaviours: endangering the health, safety and well-being of participants, staff or volunteers; stealing or damaging Humane Society or personal property; leaving the program without permission; continuing to disrupt the program and/or other participants, staff or volunteers; refusing to follow the behaviour guidelines; using profanity, vulgarity or obscenities frequently; or acting in an inappropriate manner.

The Humane Society has zero tolerance for consistent intentional behaviour that goes against the Humane Society values or any behaviour that could harm another child, an animal, a staff member or volunteer (physically or emotionally).
  • Indemnity Clause

I acknowledge and agree that my child’s participation in Paw Pals Day Damp (“the Camp”) hosted by the North Bay and District Humane Society ("the Organization") involves inherent risks, including but not limited to interaction with animals, potential exposure to allergens, scratches, bites, and unpredictable animal behavior. I voluntarily assume all such risks on behalf of my child. To the fullest extent permitted by law, I release, waive, and discharge the Organization, its directors, employees, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, injury, or damage to person or property sustained during my child's participation in the Camp. I further agree to indemnify, defend, and hold harmless the Organization and its representatives from and against any claims, liabilities, damages, losses, or expenses, including reasonable attorneys' fees, arising from my child's participation or any conduct by my child that results in harm to others. I confirm that I have read and understood this Indemnity Clause and voluntarily agree to its terms
  • Cancellation and Refund Policy

Cancellation must be made in writing two weeks in advance to the North Bay and District Humane Society. All cancellations received must be in writing by email. Refunds will not be issued without notice, with the exception that government or public health guidelines prevent day camps from operating. Refunds are not applicable for circumstances out of the control of the Humane Society, such as, weather conditions, power outages, etc. Should a camper not attend camp due to a health related issue, families will only be charged for the days they are in attendance. Remaining days will be returned to the family via camp credit, not a refund, and participants may switch their days/weeks for another day/week provided spots are available. If spots are unavailable, unused credits can be converted to a charitable donation (a tax receipt will be issued) or carried over to future camp dates.
  • Authorization

I have completed the above registration and health history form and understand the Humane Society’s procedures and permit my child to participate in the full range of Humane Society activities, except as noted. In the event of accident, injury or illness requiring immediate medical attention, I hereby give the Humane Society permission to seek medical attention by calling 911. The health history information included on this agreement is correct as far as I know. Permission is hereby given to (1) provide ongoing health care, (2) select medical personnel, and (3) order x-rays or routine tests or treatment as required by a health care professional (paramedic and/or medical doctor).

Furthermore, I have enrolled my child at the Humane Society and have read and fully understand the Humane Society Registration Agreement. I understand that the Humane Society cannot accept responsibility for the loss or damage of any personal belongings that my child may bring to camp.
