    This information is for multiple purposes. In addition to assisting with the preparations for your ordination, it will also be used for your registration with the Church Pension Group and in the Diocesan Clergy Database.
  • Please provide your name as you want it to appear on your ordination certificate and in the service leaflet. Please include any appropriate prefixes, e.g. Dr., or suffixes, e.g. JD, as they pertain to you.

  • Presenters

    Your ordination presenters are three representatives of the church at large, icons of God's presence around the ordinand. Each ordinand should have at least one lay person and one ordained person; it is expected that ordinands to the diaconate (transitional or vocational) invite a deacon to serve as one of their presenters.

    Help us recruit folks to serve at your ordination! If you can't find folks, just leave these blank. But do make an effort! These names are not published — the information is for planning purposes only.
  • Someone who will process with the gifts of bread and wine to the altar during the offertory.
  • If you know someone who is trained in how to swing a thurible, please ask them to serve at your ordination.
  • Ordinands may wish to have banners from their congregations and seminaries, with a person to carry each banner. These are not strictly necessary but they make an ordination feel more connected to our churches, institutions, and seminaries. There are MANY spaces below to include banner bearers but most people just have one or two.
  • Hymn Requests

    Note that music planning is guided by the Grace Cathedral music director, who invites requests, not all of which can be honored. If music is not in The Hymnal 1982, providing the title and tune name will make it easier to use.
