OSSL 2023-24 GAME SCHEDULE FORM
• PLEASE NOTE: GAMES CAN ONLY BE RESCHEDULED PER OSSL RULES AND WITH OSSL APPROVAL.
• In instances where games are rescheduled due to weather or tournament play, the home team is responsible for submitting new game logistics to OSSL and rescheduled games must take place by the end of the season.
• Games rescheduled within 14 days of original date may incur a $50 reschedule fee.
• If the rescheduled game date is three to six days from the time of request, the team requesting the change must pay any applicable referee scheduling fees.
