This form is for use by spectators and other individuals without a USA Fencing member profile in applying for a medical or religious exemption to vaccination requirements for USA Fencing national tournaments. Its purpose is to establish the basis for your request. Please complete all of the required fields below and also upload the appropriate exemption form and other supporting documents for review.
