Correctable Offense Form
You will use this form to submit proof of the correction for your "fix-it" offense. If you received more than one "fix-it" offense, you will need to submit one form per offense. Please call our office 812-244-2103 with questions. If there are issues with your submission, we will contact you by telephone, if possible, otherwise by US Postal Mail.

  • Please enter the UTT number or Case Number. The UTT number is located in the top right of your printed ticket. It typically begins with three zeros (000) and contains only numbers. Your case number can be found at https://Mycase.In.Gov is begins with 84H01-####-IF-######.
  • IMPORTANT PLEASE READ PRIOR TO SUBMISSION:

    KEEP THE PHOTO THE SMALLEST SIZE POSSIBLE. THERE IS A LIMIT ON FILE SIZE. Maximum size per photo is 2MB or less. Photos taken with "LIVE" selected will most likely be too large. File types of HEIC (Apple) are not accepted

    A maximum of three photos per submission is allowed.

    You may take a photo of your receipt if you purchased a bulb, paid for a repair, etc. If you complete the field for UTT/Case Number, you do NOT need to submit a copy of your ticket.

    If you are submitting proof of a headlight, brake light, tail light, turn signal, license plate light, etc. you will need to submit two photos. First photo will show the correction. The second photo will show your license plate. Obviously you cannot show a "blinking" turn light. So please submit a photo of the light as it is lit up.

  • I agree to the following terms: If I have additional charges on the case I am submitting proof of correction for, I understand I must resolve all of the charges at the same time. I will not submit any photo or other document that is not directly related to my case. I will not submit unnecessary documents or other items with this form. I could be subject to criminal charges if I submit any documentation that is a violation of the Indiana Statutes, Trial Rules, Local Court Rules or is otherwise prohibited.
  • If you are set for hearing, you must submit your proof of correction by 4pm the business day PRIOR to your hearing.  Failure to do so may result in a default judgment entered against you.
