Thank you for complying with La Plata County license ordinance. It is our goal at LPCHS to make complying with the dog licensing ordinance convenient and simple for you.

What you need to purchase a dog license
1. This form completed.
2. Upload copy of valid rabies certificate from your vet.
3. Upload proof of spay/neuter.
4. Upload valid I.D. if applicable.
5. A team member will call you for payment after application is submitted.

City residents:
Altered animal – $15 per year
Unaltered animal – $60 per year (license and permit are required)

County residents:
Altered animal – $15 per year
Unaltered animal – $30 per year

*City and county residents 60 years of age or older qualify for FREE pet licensing with proof of valid photo ID. You do not need to proceed to the payment page if you are 62 years of age or older.

*Cats are NOT required to be licensed in La Plata County.
