FSC Registration Request Form
  • This form is used to request registration changes to teams.

    All registrations during advertised registration dates must be submitted to FSC no later than 11:00pm on Wednesday prior to the weekends fixtures to be eligible for that weekend providing all checks required by FSC have been completed. All registrations after this closing point will be processed by FSC, the following week.

  • This form can be submitted for up to 15 requests.

    Please complete the name of the player, the team they are to be placed in, the team they are to be removed from (if applicable).

    If you wish to re-grade a competition player (Junior, Women's, Community League) then you must also submit a letter, stating reasons in accordance with Regulation 5.4 of the FSC Competition Rules. This is not required if the registration is for a new player.
