  • II. GMAIL CONTACTS FOR THOSE SAME 15 CLIENTS

  • 1. Check for the intake form originally taken for this client
    2. Look for names and phone numbers
    3. Add those in Gmail contacts and in CC (Daily Activities section as well as Contacts section)
  • Step 1. Open link, search client name field and check if we have a questionnaire filled out yet

    Step 2. If we have one, then (a) Look for names and phone numbers (b) Add those in Gmail contacts and in CC (Daily Activities section as well as Contacts section). *** Make sure all contacts present in CC are in GMC.***

    Step 3. If we don't have one, and we havent started services yet, chat with BAL about it.
  • Step 1. From the 2 forms you just reviewed, did it have a listed PCP?

    Step 2. Add it to the CC Assessment and add the name in the Gmail contacts Notes section (not in the phone field, just the notes [Name, Phone and Fax]. Same in the relevant section in CC

    Note: every client will be reached out to by the home office through 2 initiatives (PEAQ and Listen360) to see how happy they are

    In order to do this, we need to make sure

  • 1. If the client himself/herself has an email, it must be added in the Edit Profile section of CC

    2. In the contacts tab of every client, you must add the HCP or important family members in there and you MUST check off the box that says PRIMARY (in the relationships section)

    The home office will send out emails to the client and anyone checked off as Primary so make sure that anyone in the CC ADLs contact section that is important is also in the Contacts tab and checked off as primary (yes, you can have more than one contact checked off as primary)

    If there is not either a client email or a primary email, chat CMS

    A Primary contact is a HCP, spouse, son, daughter or guardian. Not neighbor, not accountant, not doctor, not POA unless the POA is also very involved
