VOTING INSTRUCTION FORM

  • Scheme Creditors may only submit a Voting Instruction in respect of the Scheme if their Proof of Debt is received by the Information Agent before the Record Time. If you are a Scheme Creditor and you have not submitted your Proof of Debt, please do so at here. If your Proof of Debt is submitted only after the Record Time, you may submit a Voting Instruction but your entitlement to vote at the Scheme Meeting will be at the discretion of the Chairman.

    All Voting Instructions must be submitted prior to 5.00 pm, Singapore time on 16 February 2026 (“Voting Deadline”). Please note that Voting Instructions delivered after the Voting Deadline may, at the sole discretion of the Chairman, be disregarded for voting purposes at the Scheme Meeting.

    Neither the Scheme Company, the Chairman nor the Information Agent shall recognise any sale, assignment, transfer or any sub-participation of any Scheme Claim after the Record Time for the purposes of determining entitlement to attend and vote at the Scheme Meeting.

  • SECTION 1 - PROOF OF DEBT DETAILS

    Please fill in the following details in respect of the Scheme Creditor’s Proof of Debt with the reference number assigned to your 2025 Proof of Debt (the “2025 POD Reference Number”), so that the Scheme Creditor’s Voting Instruction can be tied to the Proof of Debt with POD Reference Number.

  • For confirmation of submission and contact. Only one email address is allowed in this field.
  • Last 7 digits only. This Instruction Reference Number must match what was submitted in your POD form.
  • Last 8 digits only. This Blocking Reference Number must match what was submitted in your POD form.

  • SECTION 2 - VOTING INSTRUCTIONS

  • Please indicate your choice by making selections under A, B, or C where appropriate
  • (Please fill in the fields below if: i) the Scheme Creditor is an individual and will be attending in person; or ii) if the Scheme Creditor is a company, partnership or other non-natural Person, and a representative will be attending the Scheme Meeting to vote. In the latter case, the representative is to submit a letter of authorisation from the Scheme Creditor (being a company, partnership or other non-natural Person) to contactus@meogroup.com by 5.00 pm on 16 February 2026, (or such other time as may be determined by the Scheme Company) (“Authorisation Letter”), and present the Authorisation Letter prior to the Scheme Meeting. A sample Authorisation Letter which is provided for illustrative purposes can be obtained below.

  • SECTION 3 - ACCOUNT HOLDER CONFIRMATIONS


    The Account Holder confirms to the Scheme Company, the Scheme Manager, and the Chairman (select “yes” or “no” by selecting as appropriate for each item):

  • That in relation to the Scheme Claims identified in Section 1 of this Voting Instruction Form, the Account Holder has authority to give the voting instructions set out in Section 2 of this Voting Instruction Form, and if applicable, to nominate the Person named in Section 2 of this Voting Instruction Form to attend and speak and/or vote at the Scheme Meeting (as defined in the Explanatory Statement)

  • An Account Holder who is unable to confirm "Yes" in respect of the above should contact the Information Agent for assistance.

  • The undersigned Account Holder confirms to the Scheme Company, the Scheme Manager, and the Chairman that:

    A. the information contained in Section 3 of this Voting Instruction Form submitted herewith are true and correct; and
    B. To the best of its knowledge, since the Record Time, the Scheme Creditor has not assigned, transferred, novated, mortgaged, charged, subcontracted, declared a trust over, entered into a sub-participation in respect of, or dealt in any other manner with any of its rights or obligations arising out of or in connection with the Scheme Claims described in its Proof of Debt.

    The undersigned Scheme Creditor confirms to the Scheme Company, the Scheme Manager, and the Chairman that:

    A. The information contained in Sections 1 to 3 of this Voting Instruction Form submitted herewith are true and correct; and
    B. Since the Record Time, the Scheme Creditor has not assigned, transferred, novated, mortgaged, charged, subcontracted, declared a trust over, entered into a sub-participation in respect of, or dealt in any other manner with any of its rights or obligations arising out of or in connection with the Scheme Claims described in its Proof of Debt.
