Nursing Application for LHU Students
This form should only be completed by current or returning LHU students who wish to apply to a LHU Nursing Program.
In order to be considered for admission to the nursing program, students MUST meet the following minimum requirements:
Cumulative LHU GPA of 2.7 or higher
Academic Algebra--Grade of C or better (high school or college level)
Academic Biology with a lab component-grade of C or better (high school or college level)
Academic Chemistry with a lab component--Grade of C or better (high school or college level)
Completed Test of Essential Academic Skills (TEAS) exam score report. (The TEAS exam is offered at both the Lock Haven and Clearfield Campuses.)

Depending on when you attended LHU in the past, not all documents previously submitted may be available. The Registrar's Office keeps your file for 5 years from when you last attended so if it is 5 years or more since you last attended LHU, you will need to request your documents again and have them sent to email@example.com.
