Nursing Application for LHU Students
This form should only be completed by current or returning LHU students who wish to apply to a LHU Nursing Program.

    In order to be considered for admission to the nursing program, students MUST meet the following minimum requirements:

    Cumulative LHU GPA of 2.7 or higher

    Academic Algebra--Grade of C or better (high school or college level)

    Academic Biology with a lab component-grade of C or better (high school or college level)

    Academic Chemistry with a lab component--Grade of C or better (high school or college level)

    Completed Test of Essential Academic Skills (TEAS) exam score report. (The TEAS exam is offered at both the Lock Haven and Clearfield Campuses.)

    * Depending on when you attended LHU in the past, not all documents previously submitted may be available. The Registrar's Office keeps your file for 5 years from when you last attended so if it is 5 years or more since you last attended LHU, you will need to request your documents again and have them sent to
