YMCA of Cleveland E-Mail / PC New User Request
This form represents a request for an E-Mail, PC Login and Microsoft Teams Account for a YMCA of Greater Cleveland Employee and can only be completed by the manager / supervisor of the employee.

Please complete all required areas of this form, upon completion the YMCA IT Help Desk will be notified with your request.

Once the new user request is received by IT, employment will go through validation process with the HR Department. Once validated the account will be created and user login information will be sent back to the requester.

PLEASE NOTE: There are separate forms to request Business Cards and Name Tags (listed below), those are separate tasks that will not be fulfilled until a form is filled out for each request.
