This form must be submitted annually to verify your team's eligibility to issue Student Service (SSL) hours for volunteer activities.

For non-county pools: Proof of liability insurance must be submitted with this form as well as a description of what the students will be doing to obtain the SSL hours.

For county pools: Proof of liability insurance is not required. However, a description of what the students will be doing to obtain the SSL hours is required.
