2021-2022 Entertainment Application / Information
Please fill in the following questions for The Northwood Village Art, Dine and Design Musician and Entertainment Application / Information Request Form. This form is what we will use to advertise you playing and also use the information for your payment.

Please choose dates that you would be interested in playing and we will contact you on what is still available.

The Northwood Village Art, Dine and Design is held every Last Friday of the month from 6 pm - 9 pm

Set up is from 5 pm - 6 pm please plan to arrive in enough time to start playing at 6 pm. Check in will be at the NW Corner of Northwood Road and Spruce Ave. Set up location will be determined at the time of your check in.

Note the Art, Dine and Design event is a rain or shine event. In the case of extreme weather, the event night could be canceled for the safety for all.

We provide a 10 ' x 10' tent for this event at this time. If you wish to bring a 10' x 10' pop up tent. Your tent will be required to be weighted for safety.

We will require you to email us an invoice for your services to HenEvent@gmail.com

In consideration for my participation in the Northwood Village Art, Dine and Design, I, for myself and anyone entitled to act on my behalf, indemnify, hold harmless and release the City of West Palm Beach and the West Palm Beach Community Redevelopment Agency and their respective employees, agents, volunteers, Hennevelt’s Gallery and Gifts and the sponsors of this event from all claims or liabilities of any kind arising out of my participation in the event, even though that liability may arise out of negligence or carelessness on the part of the persons named in the waiver. I grant permission to all the foregoing entities and persons to use any photography, motion picture, recording or any other records of me in this event for legitimate purposes. I understand that all or part of the event can be canceled due to extreme weather.

    Please email an invoice to HenEvent@gmail.com no later than 3 days prior to the event date. Your payment will be available the night of the event. If you do not provide an invoice prior to the event please email your invoice asap after the event and you payment will be mailed to you with-in the week after the event.
