USWT All American Chapter Nomination

This award is designed to provide national recognition for those Women of Today chapters who have planned and accomplished a well-rounded program of projects and activities that exemplify the USWT Creed. Accomplishment of these activities will help the chapter provide the opportunities of leadership, training, community service, personal enrichment and fellowship for its members. Take pride in your accomplishments and share the completion of this All-American Chapter program with your chapter members, and submit it by May 10.

This form requires the signature of the Chapter President; be sure that he/she is present when completing it. Signatures can be done by your mouse, or your finger on a writable screen.

Responses to the online form will be viewed by the USWT President, and your state contact. Fill in as much information as you know. Upon completion of the form, click Submit; you will receive a confirmation screen if your form has been successfully submitted. (rev 2017)
