Request for Eleyo profile for facility scheduling

ISD 728 has transitioned to a new online facility scheduling system called Eleyo. You need to setup a profile to be able to submit facility requests online. To setup your profile:
• Please complete this form.
• Your request will be sent to the ISD 728 facilities office.
• Once we receive this information, we will create your Account Profile in the new software and you will receive an email to setup your password.
• Once your password is setup, you will have access to your Eleyo profile and you can submit requests, view approved usage, and pay invoices online.

FOR EXTERNAL USERS (not ISD728 Staff):
Do you schedule facilitiy reservations as a member of an organization (youth athletic associations, scouts, etc.)? If so, list the name of those organizations
FOR INTERNAL USERS (ISD728 Staff):
Do you schedule facility use for a department and/or activity/program in your school? If yes, please indicate the school and what department (i.e. band, athletics/activities, etc.)
All facility use requests will need to be made through your Account Profile at https://isd728.ce.eleyo.com. The facilities office will no longer accept paper forms, email requests or phone requests; all requests will need to be submitted through the software unless other arrangements have been made with the facilities office. More information about requesting space is provided on the facilities website (www.isd728.org/facilities).
