1.) Proof of Income

Please submit W-2 or 1099 and last current month's proof of income (i.e. paystubs, social security, alimony, child suppport, disability etc., *if self employed, submit a bank statement showing consistent deposits in account matching income reported on application, *if subsidy, provide copy of voucher. *if moving for employment reasons, please submit letter of employment.)

2.) Proof of Identity

Please submit a copy of government issued Photo ID (i.e. driver's license, passport, *if you are not a U.S. citizen, please submit a copy of visa or permanent resident card.)
