1. I do hereby enter into this contract with Sanctuary Rescue, Inc. of my own free will and agree to give foster care and shelter to the foster dog provided to me by Sanctuary Rescue, Inc. hereafter referred to as "Dog". I further know and understand that Sanctuary Rescue, Inc. does not have complete prior knowledge of Dog's health and temperament, and I shall take all necessary precautions to protect myself and others who might come into contact with Dog. *

2. I understand that Dog will be given a name by Sanctuary Rescue, Inc. and it is necessary for record keeping, appointments, public information, etc. to refer to Dog by this name only. *

3. I understand and agree that Dog is property of Sanctuary Rescue, Inc. and I do not have any ownership of Dog; all such rights remain fully and forever with Sanctuary Rescue, Inc. Dog shall not be sold, given to, placed in the care of, offered or delivered for adoption to another person, organization or any other entity including but not limited to a family member, friend, rescue facility, research facility or shelter. Dog must be returned to Sanctuary Rescue, Inc. if for any reason I can no longer care for Dog. *

4. I understand the potential for contagious illness is high in shelter animals. Therefore, I agree not to mix any of my foster animals or animals from more than one litter unless previously approved by Sanctuary Rescue, Inc. I further agree not to foster animals from any other organization while I have a Sanctuary Rescue, Inc. animal in my home unless granted specific prior approval by Sanctuary Rescue, Inc. *

5. I understand that Sanctuary Rescue, Inc. cannot and does not make any representations or warranties, either expressed or implied, concerning the temperament, habits, health, pedigree, disposition, age, sex or background of Dog and that I have no right to reimbursement of medical, legal, adoption or other professional fees. *

6. I agree to take Dog to all veterinary appointments, photography appointments, grooming appointments, scheduled adoption events, and meet-and-greet appointments. *

7. I understand that Sanctuary Rescue, Inc. reserves the right to perform routine home visits to my home and to remove Dog from my care at any time it is deemed necessary by Sanctuary Rescue, Inc. *

8. I agree that no veterinary expenses will be reimbursed unless authorized in advance by either the Vet Care Coordinator, President or Foster Liaison for Sanctuary Rescue, Inc. *

9. I agree that Sanctuary Rescue, Inc. is not responsible for any veterinary bills for resident pets. I assume responsibility for any veterinary bills that result from resident pets becoming ill or injured due to exposure to Dog. *

10. I understand and agree all dogs belonging to myself and/or being kept at my home must have up-to-date DHLPP, rabies, and bordetella vaccines and be spayed or neutered. *

11. I agree to give Dog all required medications or treatments as needed and prescribed by veterinarian. *

12. I agree that all adoption inquiries about foster animals in my care must be directed to Sanctuary Rescue, Inc. and to not make any representations to any interested parties about placing Dog in their home. Sanctuary Rescue, Inc. will handle the screening and approval or denial of all adoption applications. *

13. I understand if a Sanctuary Rescue, Inc. foster family wishes to adopt Dog, the foster family must go through Sanctuary Rescue, Inc.'s adoption program screening process. Sanctuary Rescue, Inc. reserves the right to determine final disposition of Dog. *

14. I agree to be the primary caregiver for Dog at the address on this contract. I will notify Sanctuary Rescue, Inc. in the event any change occurs in the address or telephone number listed. *

15. I understand and acknowledge that I do not have any right or authority to keep Dog, place Dog in other homes or place with other individuals unless prior permission is given in writing by Sanctuary Rescue, Inc. *

16. I understand that in the event that I am going out of town overnight and cannot care for Dog, I will contact Sanctuary Rescue, Inc. immediately so other temporary arrangements can be made. I agree that Dog cannot be placed in temporary boarding facilities or given to friends/family to temporarily care for without prior, explicit, written permission from Sanctuary Rescue, Inc. *

17. I agree, upon request, to immediately release Dog to a representative of Sanctuary Rescue, Inc. within three (3) hours of the request by a Sanctuary Rescue, Inc. executive board member. *

18. I agree to feed Dog the provided food at least twice per day and to provide an adequate supply of fresh water at all times. *

19. I agree that Dog will live inside my home and will not be isolated from human interaction. I will walk Dog on a leash or exercise Dog in a fenced area. I will never let Dog run loose or roam, keep Dog chained or tied up, keep Dog in a fenced yard when no one is at home. Dog may be crated for no longer than eight (8) hours at a time. The crate must be large enough for Dog to stand up, turn around, and lay comfortably on their side. *

20. I agree to not take Dog to dog parks or to be let off leash outside of my fenced property. *

21. I agree that if over 12 weeks of age, Dog shall wear Sanctuary Rescue I.D. tags at all times when outside the home. In the event that Dog becomes lost, I will immediately notify Sanctuary Rescue, Inc. *

22. I affirm that no member of my household has been convicted or charged with any animal welfare law violation such as neglect, cruelty, abandonment, etc. *

23. I agree that I am taking possession of Dog at my own risk, and I release Sanctuary Rescue, Inc., its officers, directors, members, employees, representatives, agents or assigns from any and all liability arising out of possession of Dog. *

24. I agree that Sanctuary Rescue, Inc. shall not be held liable should Dog bite or injure a person or another animal while in my possession. I accept full liability of Dog while in my care, recognizing the potential risks that may arise with any animal. *

25. I agree to contact foster liaison with any questions and/or concerns about Dog or the foster experience. I understand that if at any time I can no longer continue to provide proper care for Dog, I will contact foster liaison immediately, and Sanctuary Rescue, Inc. will arrange for the removal of Dog within four (4) business days. I further understand that Dog must remain safely and securely in my care until a suitable alternate arrangement can be made. *

26. I agree to ensure compliance with all applicable local ordinances and state statutes regarding possession of Dog. If I am charged with violating any such ordinance or statute while in possession of Dog, I understand that Sanctuary Rescue, Inc. is not responsible for obtaining legal counsel for me or for paying any fines or penalties assessed. *

27. I agree that Sanctuary Rescue, Inc. shall not be responsible for any bodily injuries or property damage and/or damage to other animals caused by Dog or by the actions of Dog. I hereby specifically assume sole responsibility for and agree to indemnify Sanctuary Rescue, Inc. harmless for any and all loss and expenses, including legal fees, by reason of liability imposed by the law upon Sanctuary Rescue, Inc. or any of its officers, directors, members, employees, representatives, agents or assigns because of bodily injuries or death to any person or persons including myself or those in my household or any damage to property and/or animals arising out of, or in consequence of the placement of Dog howsoever such injuries, death or damage to property and/or animals may be caused, whether or not the same may have been caused or may be alleged to have been caused by negligence of the aforementioned parties or any of their directors, members, employees, representatives, agents or assigns or any other person. *

28. I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with Sanctuary Rescue, Inc. I understand that any misrepresentations in this agreement are cause for Sanctuary Rescue, Inc. to declare this agreement in breach. *
