This information will be given to the Awards Committee to review, score and select winners, so please thoroughly answer all questions. Please pay attention to the word count limit and max points for each question to help guide your responses.

The Awards Committee selects winners using an objective points-based system based on nominee’s activity in the past 18 months. Awards will not be given to candidates who do not meet the minimum criteria threshold. This form must be completed in its entirety by midnight on Tuesday, May 12 to be considered eligible for a Leadership Award. No late questionnaires will be accepted.
