Welcome to the Care Providers of Minnesota Convention Presentation Submission Form. We encourage multiple submissions for consideration, however, please note that we do limit final selections per speaker.

It is important to fill out all required information within your proposal submission. This helps you stand out from the rest!

Suggestions for complete submissions include:


---An informative session title
---A detailed description of what the session will cover
---Three learning objectives that describe what will be gained by attending the session
---If copying and pasting information, please check that your text is not being cutoff due to character limitations

---Refrain from using bulleted lists and other special formatting in your submission

---Presentations from previous years are discouraged

Once a proposal is submitted it cannot be edited, so please do not click submit until you have finalized all information.

Terms & Conditions --- Before submitting your proposal, please review and acknowledge:


Health & Safety --- Care Providers of Minnesota (CPM) is committed to the health & safety of all convention & expo participants including registrants, exhibitors, speakers, volunteers, and staff. All convention & expo participants, including speakers, are expected to follow health & safety protocols.



Code of Conduct --- All speakers also agree to abide by the Convention code of conduct.


Presentation Submissions --- Speakers are responsible for ensuring that session proposals are received by CPM. If a confirmation email is not received, please contact Peyton Whitesitt at pwhitesitt@careproviders.org or 952-851-2494



A committee of long-term care providers and allied professionals will review all complete session proposals. Incomplete proposals will not be reviewed.



Criteria used to select proposals will include: unique and timely value of content, relevance to the needs of key constituencies of CPM, and practical, result-oriented applications.


Individuals submitting proposals will be notified of selection results via email the week of August 3, 2026. Prior to that time, Care Providers of Minnesota cannot accept phone calls regarding the status of particular proposals. Potential speakers should tentatively reserve the conference dates of November 9th, 10th & 11th , 2026 to ensure availability.



Speaking Elements --- Speakers who are new to Care Providers of Minnesota may be asked to provide professional references. Breakout session speakers will not be compensated and are responsible for their own convention-related expenses. However, as a thank you, speakers are invited to fully participate in the Convention & Exposition event with a complimentary registration.






To maximize educational benefit, we request that speakers create handouts (a PowerPoint presentation for example) for their sessions. Care Providers of Minnesota will create PDFs of materials to share online and on the APP. Handouts should be emailed in an electronic format (MS Word, PowerPoint, etc.) and submitted NO LATER THAN October 1, 2026. Speakers must contact Peyton Whitesitt at pwhitesitt@careproviders.org or 952-851-2494 in advance if they are unable to meet the deadline.




Care Providers of Minnesota reserves the right to edit session descriptions, objectives, bios, handouts, and any other submitted information for space and content.
Convention education sessions are non-commercial forums, and under no circumstances should the Association’s podium be used as a place for direct promotion of a product, service, or for monetary self-interest.


Speakers represent and warrant to Care Providers of Minnesota that they own (or have obtained valid permission to use) all copyrights and other intellectual property in the handouts provided and that none of it will infringe the rights of any third party. If Speaker has obtained valid permission to use any copyrighted material or other intellectual property, they will clearly identify such material and its third-party owner. Speakers are responsible for the accuracy of the information.



By presenting at the convention, speakers acknowledge and agree that photographs may be taken by Care Providers of Minnesota or parties acting on behalf of Care Providers of Minnesota, and that these photographs may be used by CPM in any media now known or later invented for any purpose related to Care Providers of Minnesota’s mission, including educational, promotional, and awareness-related use.




Cancellation --- Speakers must inform Care Providers of Minnesota within 30 days before their scheduled presentation date if, for any reason, the session will not be presented and make every effort to find a suitable replacement for their presentation time and topic.

  • SESSION TITLE
    Session titles help attendees determine which sessions to attend. Titles should describe, to the best extent possible, what the session will be about.

  • AUDIENCE & ENGAGEMENT

  • Care Providers of Minnesota provides the following standard AV for each room:
    LCD projector
    Screen
    Podium with wired microphone
    One wireless microphone
    Appropriate sound system
    Internet access

There may be changes with COVID protocols between now and convention; rest assured, Care Providers of Minnesota follow all CDC COVID guidelines for large indoor gatherings in effect at the time of the convention.
