Neisha's Dance & Music Academy Summer Camp Waiver
I hereby acknowledge and agree to the following terms and conditions for my child to participate in Summer Camp at Neisha's Dance & Music Academy, scheduled from June 17th to June 21st, 2024:
1. I understand that participation in the camp involves physical activity and there are inherent risks associated with such activities.
2. I certify that my child is in good physical health and able to participate in the activities offered at the camp.
3. I authorize Neisha's Dance & Music Academy staff to seek medical treatment for my child in case of an emergency.
4. I release Neisha's Dance & Music Academy, its staff, instructors, and affiliates from any liability for injuries or accidents that may occur during the camp.
5. I agree to abide by all rules and regulations set forth by Neisha's Dance & Music Academy for the duration of the camp.
I have read and understood the terms of this waiver and voluntarily agree to its contents.
