By signing this permission slip, you acknowledge that your child may actively participate in Twirls and Tiaras Spa Party Bus activities which may include the following:

Your child will be receiving manicure and pedicure services including but not limited to the use of whirlpool footbaths, sea salt soak, sugar/salt scrubs, scented lotions/oils and etc. Your child may do a spa facial with the use of facial cleanser, masque, cucumber eye pads, exfoliator, honey, yogurt and/or moisturizer. Your child may also have a makeup application with the use of eyeshadow, blush and/or lip gloss. The use of hair accessories, party props, clothing and etc. will be used. Girls will have their photos taken and/or video recordings during the party. Photos may be uploaded to social media and/or online gallery for promotional purposes. Twirls and Tiaras Spa Party Bus cannot be held responsible for how photos are shared. During the event, your child may be going up and down stairs to get rid in and out of the bus. Twirls and Tiaras Spa Party Bus is not responsible for any damages, accidents or injuries that may incur while under our supervision.

By signing this permission slip, you assume all risk and hazards incidental to participating in the spa/party activities and waive, release, absolve, and indemnity and agree to protect, defend and hold harmless Twirls and Tiaras Spa Party Bus, their owner, supervisors, participants, hosts, and drivers for any claim arising out of any injury/medical injury to your child to the fullest extent. You will further grant permission to use without recompense, any photographs, video/audio recordings, or any other recordings for promotion of Twirls and Tiaras Spa Party Bus.
