I agree to participate in all trainings provided by AFLV and/or the Co-Lead Facilitators.
I agree not to reprint, reproduce, or reuse the UIFI curriculum—or any portion of it—without the express written authorization of AFLV.
I agree to fully participate in all UIFI activities and remain at the host site for the entire duration of the program.
I understand that UIFI is a substance-free program, and I agree not to bring, use, or consume alcohol or drugs at any time during my participation in the program.
