Please submit ONE application per student or ensemble. Please fill out the form completely.

The fee is $25 per student or ensemble payable to MTAC San Fernando East Valley (SFEV) Branch. Payments may be made by Credit Card, PayPal or Check.

Compositions must have been written after 1920 to be eligible. However, all compositions by Bartok, Stravinsky and Prokofiev are acceptable regardless of their date of composition. Please note that the copyright date is NOT the same as the date of the composition. Be sure to research the actual composition date to ensure eligibility
