CSA pick up will be on Tuesdays only starting Tuesday October 15th and ending Tuesday November 12th. Please note the pick up time from 3pm-5pm!

• Bag pick up is at The Leaf, 65 Blueberry Lane, Southington Ct

• The 5 week CSA Extension will be a traditional CSA, we will have your produce in a bag and ready for you at the time of pick up.

• Sign up and Payment can be made here or by cash/check. If paying cash/check, please bring payment at the first pickup. All checks can be made out to "The LEAF".

• Some of the produce we will have available for the 5 week CSA extension are, Winter Squash, Sugar Pumpkins, Tomatoes, Cucumbers, Lettuce, Spaghetti Squash, Sweet Potatoes, Beets, Brussel Sprouts, Broccoli, Cauliflower, Eggplant, Peppers, Onions, Radishes, Kale, Swiss Chard, Pac Choi, Mustard Greens, Kohlrabi, Fennel, Turnips, Garlic, Carrots, Apples
I understand that my full payment entitles me to a weekly share of the produce of which I have indicated on this form. I understand that Lewis Educational Agriculture Farm will do its best to supply as much produce as possible and may have to bring in produce from other local farms to help supply product to fulfill my share. I understand that there are risks involved in agriculture such as weather and crop failure. I understand as a member of this CSA that I share in the risk and accept unforeseen outcomes. I understand if I or another person picking up for me is unable to pick up my CSA share it will be forfeited and donated to Southington Social Services or Bread for Life. I understand that failure to pick up my share during the set pick up time without prior notification will result in loss of my share for the week and it will be donated. I understand that my payment is nonrefundable, but is transferable to another person by giving Lewis Educational Agriculture Farm 1 week written notification prior to transfer. *
