-This voucher is for Ohio residents only. Each person may only receive ONE voucher code.

- The voucher is good for $20 off each of the 4 sections of the GED Exam. You will need to pay $10 per exam.

-In order to qualify for the $80 GED voucher, you need to complete all information on this form. Incomplete forms will not be processed.

- When you "SUBMIT" a complete form, you will receive an email within 2 WEEKS from TLISKA@polaris.edu with your voucher number and instructions for redemption.

- After you've received your voucher, schedule and pay for your test at www.GED.com

-Voucher codes expire on June 30, 2022, so plan accordingly. If you do not complete all 4 sections of the exam prior to June 30, 2023, you will need to pay full-price for future sections of the exam.

-If you do not pass a section, you are allowed to retake it up to 2 times for only $10 per retake. You do not need a voucher code for this discount.
