I have accepted the quote given to me by TRAVEL BY FOOTE and authorize TRAVEL BY FOOTE usage of said credit card on my behalf for services quoted and accepted. I understand this is a disclosure of business practices and permissive payment agreement for the purchase of travel services. I agree that payment may be made direct to selected travel suppliers, as stated below, on my behalf by TRAVEL BY FOOTE; paid in prorated amounts not to exceed the sum total of approved charges. I understand Debit Cards often impose "Daily Charge Limits" and multiple payments may be needed to complete the full balance due. I am aware of any potential Cancellation, Refund and Surcharge Policies and Fees by suppliers and agree not to dispute or charge back any of the below signed for and acknowledged charges without proper course of action. TRAVEL BY FOOTE will not release personal information to ANY parties not listed on your finalized Travel Quote and will apply due diligence in maintaining records in a secure, safe location.

DOCUMENTATION: U.S. citizens travelling to any destination outside of the United States will be required to present a valid U.S. passport. Passports must be valid for 6 months past the date of travel. Some countries require a visa for transit or entry. Passengers are responsible to ensure that they have all the proper documents for entry. All names on documents must match the legal name on your photo I.D., and travel document information must match tickets. Please check the State Department website at http://travel.state.gov for further information. Immunizations may also be required. Failure to comply with these regulations may result in denied boarding, denied entry, and/or government imposed fines. If you are a citizen of another country, there may be additional requirements. Check with the nearest consulate or embassy of the destination you are travelling to and find out the entry requirements for non-U.S. citizens

SPECIAL NOTE: If you have any legal issues that would prevent you from traveling outside of the US, it is YOUR responsibility to verify your ability to travel. TRAVEL BY FOOTE will not be responsible for any denied boarding or entry due to legal matters.

TRAVEL BY FOOTE offers retail travel services to customers, which are provided by separate and independent vendors of travel services. TRAVEL BY FOOTE does not operate, control, or otherwise provide the services of the independent travel vendors. Hence, customer agrees that TRAVEL BY FOOTE acts only as agent for the client in acquiring transportation, hotel accommodations, sightseeing and other privileges, or services for the clients’ benefit, and on the express condition that TRAVEL BY FOOTE shall not be responsible for any loss, accident, injury, delay, defect, omission or irregularity which may occur or be occasioned, whether by reason of any act, negligence or default of any company or person engaged in or responsible for carrying out any of the arrangements, or otherwise in connection therewith. Customer agrees and understands that TRAVEL BY FOOTE is not responsible for any damages, direct or consequential, which may arise as a result of acts of God, social or labor unrest, war, political or national strike, terrorist activities, mechanical failures, climatic conditions, pandemics, or any other actions or omissions beyond TRAVEL BY FOOTE’s control. In the event an airline or travel vendor declares bankruptcy, it is not obligated to carry you or to provide funds. Money given to my agency immediately becomes the property of the airline or travel vendor as required by law. TRAVEL BY FOOTE is not allowed to provide refunds for vendors who have declared bankruptcy. If an airline or travel vendor declares bankruptcy, it might continue services or stop completely. Other airlines or travel vendors might provide alternative services under the limited circumstances.
