This is a disclosure of business practices and agreement for the purchase of travel services. I agree that payments may be made direct to selected travel suppliers on my behalf by Epiphany Travel Agency.

Epiphany Travel Agency agrees to provide services as follows:
• Agrees to facilitate the booking of travel (chartered bus, hotel, meals, attraction tickets).
• Collect deposits and apply payments on your behalf.
• Agrees to provide support in case of any issues that may occur before, during and/or after travel.

Participant(s) agrees to the following:
• Be attentive to consultant communications.
• Complete travel specific duties.
• Make all payments by the stated deadlines.
