2024 Award Candidate Questionnaire

Congratulations on your nomination for one of FSAE's 2024 Leadership Awards! We need more information from you to complete the nomination process.

This information will be given to the Awards Committee to review, score and select winners, so please thoroughly answer all questions with as much specific details as you can include. Please pay attention to the word limit and max points available for each question to help guide your responses.

The Awards Committee selects winners using an objective points-based system based on nominee’s activity in the past 18 months. Awards will not be given to candidates who do not meet the minimum criteria threshold. This form must be completed in its entirety by midnight on Monday, May 13 to be considered eligible for a Leadership Award. No late questionnaires will be accepted.
