Baptism Application Form
Parish of St John Paul II, Eccles

The application must come from a parent who is a Roman Catholic who should normally reside within the parish boundary of the parish of St John Paul II, Eccles. We would normally request that the family are familiar with the life of the parish by attending Mass for a suitable period before the celebration of Baptism, but we recognise that the current health crisis may restrict this involvement to attending Masses online.

Church teaching indicates that:

“The parents of an infant who is to be baptised and likewise those who are to undertake the office of sponsor are to be properly instructed in the meaning of this sacrament and the obligations which are attached to it; personally or through others the pastor is to see to it that the parents are properly formed by pastoral direction and by common prayer, gathering several families together where possible visiting them”.

“There is to be a founded hope that the infant will be brought up in the Roman Catholic religion; if such hope is altogether lacking, the baptism is to be put off according to the prescription of particular law and the parents are to be informed of the reason”.

  • We need your email address in order to acknowledge receipt of your application, and to keep in touch as the programme of preparation progresses.
  • GODPARENTS

    1. Should be not less than 16 yrs of age.

    2. Should be a Roman Catholic who has been Confirmed and who lives a life of faith which befits the role to be undertaken (a practicing Roman Catholic).

    3. However, one of the Godparents may be a baptised member of another Christian Church in company with a Roman Catholic

    A date will be set for the baptism in good time for you to invite your guests.

    HOWEVER please keep in touch with us, otherwise the form will not be retained beyond six months.

    We have thought very carefully about the responsibility we are undertaking and the responsibilities that come with Baptism. We promise to bring our child up in the practice of the faith and to bring our child to Mass and the Sacraments once circumstances permit.
