    Congratulations! We are pleased to invite you to sing with The Jacksonville Children's Chorus this season.

    Please complete the following:

    1. This Online Registration Form
    (Form must be 100% complete for a singer to be registered)

    2. Submit payment for your singer's registration fee. Registration is NOT complete until payment has been received.

    3. Complete a 2026-2027 Medical Release Form – notary required.

    4. Apply for a Scholarship, if applicable, by completing the 2026-2027 Scholarship Application.
