The special event insurance carrier requires that all participating vendors, breweries and contractors name the following six entities as an "additional insured" for the 25th Annual Mammoth Festival of Beers and Bluesapalooza:

Please make the expiration date on or after 8/9/2021. Insurance liability limits must be at least $1,000,000 each occurrence and $2,000,000 General Aggregate. A copy of your insurance certificate is required for Vendor participation, so please forward your updated forms to Jim at
