By completing and returning this form, I grant permission for the Australian Calisthenic Federation Ltd (ACF) to:

Use photos, videos and audio recordings of the performer named below including:

* Their first name and last name initial
* The name of their Club and State in connection with the above
* The name of the photographer/videographer, coach, costume designer if supplied

And to distribute them in the following locations:

* Printed publications e.g., newsletters, promotional material
* Publicly accessible social media accounts
* Publicly accessible websites
* Any other form of print or online media

This permission will continue until I revoke permission in writing to: marketing@calisthenicsaustralia.com.au.

Please note:

* Items might not appear in exactly the form in which they have been submitted and not every item for which permission is granted will be used.
* Where permission is revoked, every effort will be made to remove relevant media from distribution, however this may not be possible or practical in some situations.
