Monsignor Reynolds Charity Challenge

Instructions:
- A single player can register for herself or himself
- Team Leaders (2-4 players) must register all players on their team and pay for all players (no individual payments by team members)
- Everyone registering and paying by February 12 qualifies for a $20 per player discount
- If the names of all Team players are not known by the Team Leader at the time of registration, enter TBD in the Last Name and leave other information blank
