This is for children grades 1-5.
Please use one form per child. Register for as many camps as you like. If space remains available, you may register for additional camp(s) later by using this same form.

Payment in full is required at registration. Refunds will be given in full for for cancellations made two weeks prior to camp date.

Please have payment and health insurance information ready before completing this form.

  • If different from above, please list name and email of person who should receive camp information from the Camp Director the week before camp begins. (This would normally be the person dropping off the child each day.)
  • Please let us know whether you learned about our school break Creature Camp through local media, from friends, from your child's school, or elsewhere, or if this is a returning camper. Thank you!

  • The Creature Conservancy staff will be taking photographs of camp activities that may be used in promotional materials. I hereby grant permission for my child's image to be included in camp photographs.
